What costs are associated with this proposal? $500 – To adjust the backend of the form.

Additional Pertinent Information: (if none state "none") The current NOP reads for both a Collegiate and an Initiate Risk Management Agreement (Forms 20/21 and 20A/21A, respectively). Each term requires both Brothers and Initiates to sign a short form online with a click, and many Brothers and Chapters have given feedback on frustration throughout this process. For the past year, Staff has worked in conjunction with James R Favor and Co. to create a template membership agreement, which ultimately will replace the Risk Management Agreement.

Ideally, Forms 20/20A/21/21A will all be collectively known as “Form 20: Membership Agreement.” Signing this form once (either virtually or a hard copy) would capture the agreement for the lifetime of membership in Phi Sigma Pi to refrain from (in short) allowing hazing, harassment, etc. to occur with their knowledge in any program, event, or otherwise.

As a new form, the Form 20 would need to be signed in Fall 2013 by all Brothers and Initiates. Rolling into Winter/Spring 2014, only new Initiates and any Brothers not captured in the Fall 2013 Form 20 (i.e. Brothers returning from National inactivity) would be required to sign the form. Small adjustments to the NOP in the sections pertaining to Member Delinquencies (under Chapter Requirements) and Initiate Failure to sign Form 20 (under Initiation Requirements) would cover our needs with the changing forms for the 2013-2014 academic year. Additional adjustments in these sections would be necessary leading into Fall 2014, as the vast majority of Brothers will have signed the new Form 20: Membership Agreement in the previous academic year.

Proposal

I move to amend the National Operating Policies such that the following sections would read,

“Member Delinquencies (form 20/21Form 20) In the event a Collegiate Member fails to sign his/her form 20/21 Risk Management Agreement Form 20 Membership Agreement by the due date, the form will be considered late. The National Staff shall provide outreach in an effort to get the form signed. Chapters are responsible for working with their members to complete each form.

If the form is not signed within two weeks following the due date, the National Council shall review the situation for consideration of additional disciplinary action.

Brothers who have signed the Form 20 Membership Agreement in Fall 2013 (as Initiates or Brothers during that academic term) will not be required to sign Form 20 in any subsequent academic terms. “

“INITIATION REQUIREMENTS Initiate Failure to Sign Form 20A/21A In the event an Initiate fails to sign the Form 20form 20A/21A Risk Management Membership Agreement by the established due date, the National Staff will provide outreach in an effort to get the form signed.

If the Initiate has not signed the form Form 20A/21A one within two weeks after following the due date, the Initiate will be considered to have resigned from the Chapter’s Initiation Program. Any Initiate who resigns from the Initiation Program under this provision shall not be eligible to restart the Initiation Program until the following academic termsemester/quarter.”
